Summary:

The Program Facilitator’s foremost responsibility is the safety and welfare of all residents, and the daily facilitation of the Green Hill program. The Program Facilitator is the primary leader of the Green Hill therapeutic community and must conduct himself in a professional manner. Setting a positive example of appropriate behavior, industriousness, punctuality, and attention to detail is expected. The Program Facilitator will work with the clinical team, Program Coordinator and Executive Director to advance positive therapeutic outcomes for each individual client and the Green Hill sober-living program.

Our ideal candidate has personal experience in recovery, is interested in making helping people a lifetime pursuit and is able to think on their feet while having fun. Both full and part-time positions are available, and a variety of advancement opportunities are available to qualified candidates. 

Duties and Responsibilities

  1. Facilitating daily transitional living programing as identified by the weekly transitional living program schedule. 
  2. Providing and managing client transport to GHR/medical appointments and group activities; limited personal transport may be accommodated when schedule permits.
  3. Following and completing the PF shift responsibility checklist for each respective shift worked. 
  4. Assists residents in developing independent living skills i.e. cooking, laundry, budgeting, personal organization, time-management etc. Facilitating and documenting daily medication observation.

Minimum Qualifications

  • Valid driver’s license and clean driving record.  
  • Ability to pass Green Hill Recovery’s background check. 
  • Ability to administer urinalysis.
  • Ability to lift and manipulate 40 lbs worth of weight

Summary:
The Green Hill Recovery Primary Therapist will serve as a member of a multidisciplinary team to provide professional level counseling to clients and their families under the direction of the Clinical Director. As a member of the treatment team, they are responsible to provide a broad range of treatment services including individual, group, and family therapy, crisis intervention as well as completing assessments and reports. They will work closely with guardians, families, and referral sources to provide quality care to those served. The Primary Therapist will also contribute to the development of clinical curriculum utilized throughout the Green Hill Recovery program.

Duties and Responsibilities

  1. Leads individual, group and family therapy sessions with both Green Hill Recovery’s Community Program and Transitional Living clients 
  2. Utilizes behavioral management strategies, including but not limited to Cognitive Behavioral, Motivational Interviewing, and Community Reinforcement Approaches
  3. Effectively and compassionately manages a minimum caseload of 8 GHR clients at any given time    
  4. Completes Master Treatment Plans and conducts routine treatment plan reviews with each client according to GHR’s policy and procedure
  5. Completes Transitions Plans and Discharge Summaries with each client according to GHR’s policy and procedure
  6. Provides weekly (or as needed) Family Therapy utilizing Green Hill’s Family Program material  
  7. Provides clinical case management including: correspondence with families, outside medical professionals, referring professionals, members of the judicial system, insurance companies and other stakeholders as needed  
  8. Responds to email/text/phone communication within 24 business hours
  9. Completes timely and accurate clinical charting and progress notes according to GHR’s policy and procedure 
  10. Conducts urine drug screens as clinically fit
  11. Attends and participates in weekly clinical meetings including: individual supervision with the CD, and weekly treatment team clinical meetings; Brings new ideas and solutions to identified problems
  12. Conducts clinical assessments and assigns diagnoses for newly admitted clients 
  13. Collaborates with necessary parties (i.e., utilization management) on clinical justification for continuing care needs
  14.  Assists with crisis management and on-call support 
  15.  Assists in the development and facilitation of GHR’s experiential programing 
  16.  Assists in the development of clinical procedures.
  17.  Pursues professional development opportunities including mandated continuing education and maintenance of proper licensure  

Minimum Qualifications

  • Current licensure: Dual licensure as LCAS and/or LCMHC, LMFT, LCSW
  • Master’s Degree in human services related field
  • Valid driver’s license and clean driving record
  • Ability to pass Green Hill Recovery’s background check

Competencies

  • Experience working with complex, dually diagnosed clients who present with needs related to emotional regulation, anger management, family dysfunction patterns, and substance use or other process addictions
  • Experience and knowledge based skills used to conduct family therapy with a central focus on family structure with an addiction disorder present
  • Ability to perform a comprehensive clinical assessment and use of the ASAM PPR-2 to justify client level of care clinical needs
  • Ability to work interpersonally and utilize Motivational Interviewing with clients
  • Exceptional organizational skills and attention to detail
  • Excellent oral and written communication skills
  • Must be approachable and friendly and be able to exude empathy and control in communication with clients and parents 
  • Must have strong problem-solving skills and strong service skills
  • Have the flexibility to respond quickly and appropriately to changing circumstances and clients’ needs. Ability to show initiative and take the lead on projects, while also being able to take direction when needed

Summary:
The Clinical Case Manager supports Green Hill Recovery’s clinical and programmatic operations by facilitating weekly case management for all Green Hill clients. A Clinical Case Manager will take the lead in helping the client with daily life skills and role modeling management of independent living skills. This individual also holds clients accountable for their academic, vocational, and medication management needs.

Duties and Responsibilities

  1. Meets weekly with each resident on an individual basis to ensure clinical, academic and life skill action items are being met and properly prioritized.
  2. Identifies the independent living skill needs with each client, and assists in devising the client’s Master Treatment Plan
  3. Evaluates clients’ independent living skills and assists in therapeutic interventions around these needs
  4. Assesses needs in the following areas and formulates a detailed plan to address each: A. Academic B. Vocational C. Volunteer opportunities
  5. Documents outcome of case management meetings in Green Hill Recovery’s EMR and in weekly case management update to staff.
  6. Manages client logistics ensuring client follow-up regarding medical appointments, GHR activities and travel.
  7. Attends weekly clinical team meeting(s) to provide updates to all present clinical staff regarding client progress
  8. Participates in GHR’s medication process, and reports incidents to ED and CD as they arise
  9. Facilitates filling of client medications.
  10. Oversees and conducts weekly medication audit with identified Program Facilitator
  11. Meets weekly with each resident taking prescription medication and assists them in making medical appointments and their prescriptions are being refilled in a timely manner.
  12. Responsible for execution of curriculum related to academic and vocational
  13. Reports medication changes to the clinical and direct care teams.
  14. Assists in the facilitation of resident intakes and admissions.
  15. Assists in the development and execution of program policies and procedures.
  16. Assists the ED and CD with special projects and other duties as assigned.

Minimum Qualifications

  • Bachelor’s degree preferred
  • Valid driver’s license and clean driving record.
  • Ability to pass Green Hill Recovery’s background check and drug screen

Competencies

  • General understanding of substance use disorders and mental health treatment
  • Ability to work interpersonally and motivate clients with co-occurring disorders.
